Final Transcript Request and Checkout Form
All seniors are REQUIRED to complete a final check-out form and transcript request. New this year, the form is online! Seniors should complete this online form by May 10. The link to the form is on the front page of the Roswell website, the Student Services website, has been sent out via the PTSA newsletter and is available directly at: https://goo.gl/forms/AVoeE5tXC5AMMGMs1 . The check-out form will ask students to indicate where they need their final transcript sent (if applicable). In addition to completing the online request form, students are also responsible for requesting their transcript through Parchment or Common Application. Failure to complete this form will result in delays in processing final transcript requests which can impact post-secodary enrollment.
Parchment.com – this is where a majority of student transcripts are processed. If they requested one be sent previously, they need to request it again so we can send an official transcript with proof of graduation. They must check “hold for grades” so we know to send it only once final grades are posted.
Common Application – students would only select this option if the college or university they plan to attend is on Common Application. We will send their final transcript directly to the college via Common App.
NCAA – scholarship student athletes need to select this option (in addition to either Parchment or Common App) so the NCAA can verify final eligibility and graduation.
If the student is undecided as of May 10th, it is their responsibility to request a transcript from Student Services by May 31st or no final transcript will be sent, which can delay college or university enrollment.
Senior Exit Survey
Fulton County requires every 12th grade student to complete the Senior Exit Survey regarding plans after graduation. Please click the link below and complete this survey by April 22nd. Failure to complete this required survey can delay participation in graduation activities. Click the link below to get started!
